The ketubah that Sandi holds is the traditional Jewish wedding contract. (Well, not so traditional in Jim & Sandi's case.) Sandi is holding the ketubah aloft to show that she understands and agrees with all it says.
Sitting in front of them are two of the witnesses, David Merfeld and Bonnie Friedman, waiting to sign. In a borrowing from Friends' tradition (not to mention friends' ceremony), all the wedding guests were later invited to sign.
